No one can predict cancer, TBI, SCI, mental health impairment, blindness, etc. There are a number of conditions that can keep you from working as a physician. Unless you are already independently wealthy, it just makes sense to get disability insurance

Your focused on the "own-occ" portion but DI is still useful for any number of reasons.
But I would not dismiss the "own-occ" portion. The husband of my wife's best friend was in a ski accident and due to his TBI had long standing issues with complex reasoning that prevented him from his job as a bank attorney. Even though he had no physical limitations, he couldn't maintain the focus, accuracy, and speed needed to be adequate in his job. Only after a lot of cognitive rehab was he able to make it back to work and luckily he had own-occ on his DI for his family.
Additionally, you may survive a bad MVC mentally intact but not be able to physically manage a full day of clinic for a long while even with adaptive technology. Having a legit DI policy is important in that scenario too. Even if you eventually do get back to a full clinical load. The long term DI policy buys you that time to recover.
Lastly, if you do have that more devastating injury that prevents you from normal ADLs and cognition, even though it'll still be a lower quality of life, you never reached a critical status/life threatening where there was a decision to make, you still need that underlying DI policy in place.

or a TBI (happened to a doc I know). Bad enough to not be able to practice but still able to somewhat function.
