I used his image, and this is basically how I added it to unraid as a custom docker container.
Docker tab -> Add Container
_toggle advanced to see all the options_

Network Type: br0
Fixed IP address (optional): static ip -- you could try to leave this blank and let DHCP get it, but i recommend setting this. Also, at the bottom you can toggle "Show docker allocations" to see all your custom IPs to make sure you're not picking one already in use.

Then add a Path for persistent storage: (towards the bottom of the add container screen)

Name: bitmonero (or whatever you want)
Container Path: /home/monero
Host Path: /mnt/user/appdata/monerod <-- you will need to create `monerod` on the host via SSH.

As for port forwarding: The reason I changed the network type from bridge to br0 is to make this easier. Now you have a dedicated IP, and you can just login to your router and create a port forward (or DNAT) rule to send traffic destined for port 18089 to the IP of your container.
