“Move to the US” is pretty shitty advice if Toronto is where you actually want to live (and given current events). If living there is right for you, that’s great. The ceiling may be lower than in the US but you can still do very well if you’re intentional about how you structure your career.
From an earnings standpoint, I’d look hard at a few things:
* Learn the OHIP fee schedule inside and out. Know which types of visits, procedures, add-ons, premiums, and combinations of services actually pay well relative to the time involved. Ontario’s physician fee schedule also changes over time, so staying on top of billing rules matters more than people think.
* Think about your mix of work, not just your hourly rate. Clinic, inpatient consults, procedures, call, diagnostic interpretation, and academic/hospital work can have very different economics. Figure out which combination gives you the best $/hour without making your life miserable.
* If you’re building a clinic, keep overhead low at first. Don’t rent a huge space and hire a bunch of staff before you have the volume. Add capacity as referrals grow.
* Get very good at billing. A surprising amount of money gets left on the table through missed codes, rejected claims, poor documentation, or just not understanding what can legitimately be billed. Use good billing software and have someone competent review your remittance statements.
* Incorporate when it makes sense and get an accountant who works almost exclusively with physicians. The point isn’t that incorporation magically lowers all your taxes, but it can give you much more flexibility if you’re earning substantially more than you need to spend personally and can leave money inside the corporation.
* Be strategic about referral relationships. If you’re efficient, communicate well with referring physicians, and can see patients reasonably quickly, your clinic can fill up very fast. Volume matters a lot in a fee-for-service system.
* Talk to high-earning cardiologists in Toronto and ask what they actually do. General cardiology vs imaging vs interventional vs EP, how much call they take, what they bill annually, what their overhead is, and what they would do differently if they were starting again. That information is probably worth more than anything you’ll get from people just saying “go to America.”
I’d also avoid optimizing purely for gross billings. Someone billing $900k with $300k of overhead and no free time may be worse off than someone billing $700k with minimal overhead and a much better schedule.
If Toronto is home and that’s where you want to be, I’d build the best setup you can there. It may be harder to maximize earnings than in some US markets, but it absolutely can be done.

Hi! If this isnt allowed please let me know and I will take the post down. Ive had this sitting in a box for 20 years. Some are stamped EPBM WR CANADA 591 (sugar bowl, gravy? boat, small teapot), others arent stamped at all (large platter and ornate pitcher with eagle head? with 2 enamel? beads in the handle), and one is a bowl stamped ALVIN EP RC135.
Ive been wondering if I should hold or sell/donate for 20 years! Im moving soon and need the space. Help a lost lady out? Thank you silverbugs!

I am gonna laugh if the EC/EP decides they need to stop outflows of cash from Europe and limits retail investment in foreign equities.
European economies have SEVERELY lagged the US and China. I’ve been visiting for years and every year my salary buys more while every year Europeans can afford less.
