The thing that made this clean for me: stop logging into each sub-account separately. Client Portal lets you link accounts under one username, then one gateway session covers all of them, one IB Key push at login, and you route per book with the account field and client ids in the API. The "which 2FA request is this" problem disappears because there's only ever one request.
If the accounts have to stay separate logins — different entities or regions — the workaround is gateway auto-restart: it restarts on schedule without a manual re-auth, so the 2FA pain only shows up when a session truly dies. People run that for months at a time.
Multiple hardware keys is DSC+ territory like you said, so that's out below the threshold. Between linking and auto-restart I never found the daily restart ritual worth keeping.

Hi,
I am trading multiple books systematically using different sub-account. One of the challenges is that I do not see what 2FA request belongs to what account.
That means I restart each gateway one-after-another on a daily basis to re-authenticate each account/sub-account.
Maybe multiple HW OTP keys would work here?
Unfortunately it's still below the DSC+ threshold so I would appreciate any recommendations how to set this up a bit cleaner.
