We had our son PGY1. And a second one PGY3. A third the year he started practice. He was the doc, I was the banker, keeping us afloat while he did med school.
He did family med, and many of his colleagues also had kids during residency, so it was a really great community situation, where nobody had enough money to host a party so we had gatherings which were BYOB and also BYO meat for the barbecue. We all brought the babies and toddlers.
It worked out for us. I think I would do it again, but given a LOT more thought to locating near extended family, because caring for tiny kids when your spouse is on call 1 in 3 nights is freaking hard (he would sleep night 2, so he was really only "around" 1/3 of the time.
2 of those 3 kids also became family docs, but chose to delay starting a family until they were in practice. I can't really argue with their logic.
