After doing some research myself and spending OVER A MONTH on the chat with Revolut representatives and after a few trips to Laos, here's the answer:
Long story short, it's VISA, not Revolut per se. Visa gives horrible margins when you exchange by withdrawing cash from an ATM. However, you will get a better rate in Laos if you have a Mastercard.
As for ATM access fees, these are unavoidable in Laos. All ATMs charge at least 30.000. BCEL will let you withdraw 2 mil LAK for 30.000 using a mastercard whereas Indochina Bank will let you withdraw 3 mil LAK for 40.000 but only for VISA (at worse rates).
